The Virtua Heart Failure Program in Moorestown is a community-based program with offices located throughout South Jersey. The team’s proven education and treatment protocols are designed to help patients manage their congestive heart failure (CHF) and avoid unnecessary hospital visits. Patients actively work with their physician, nurse practitioners, nurses and family members to manage their disease through medication, diet changes and appropriate activity. Through this team approach, we enhance the well-being of our patients, promote optimal heart function and reduce CHF symptoms.
